Donation Request Form

Union Bank & Trust has a long-standing commitment to helping our customers, each other, and our communities. Because of that, UBT Foundation was established to serve a leadership role in supporting a wide variety of non-profits that address needs and make our communities stronger. To request support for your 501 (c)(3), complete the form below. Please ensure the name of your organization matches the registration of the EIN provided. We ask that each organization limit your requests to one per calendar year. Requests will be reviewed on a monthly basis. Requests must be submitted at least 10 days prior to our meeting. No meetings will be held in December. If you need funding by the end of the year, you must submit your request by November 15th.

All requests are restricted to nonprofit, tax-exempt organizations under Section 501 (c)(3). We focus our contributions in three areas: Community/Social Service, Education, and the Arts. Please note: individual, church, and political campaign requests are NOT funded. We are committed to supporting the communities we serve. These communities are located in Nebraska, Kansas, and Colorado. Requests from outside of these communities are generally NOT funded. 

As a 501(c)(3) private foundation, we are committed to maintaining compliance with IRS regulations governing charitable giving. In accordance with these regulations, our foundation cannot receive or accept any ancillary benefits in connection with the grants or charitable contributions we make. This includes, but is not limited to:

  • Complimentary event tickets
  • Tables or seats at fundraising events
  • Advertising packages
  • Goods or services of any kind provided in exchange for a charitable gift

When our foundation makes a grant, it must be strictly charitable and cannot be tied to any benefits or privileges that carry monetary value. However, we understand that fundraising events may offer tables, tickets, or other benefits that are appealing to individuals or businesses connected with our community. Those individuals or businesses are welcome to separately purchase tables, tickets, or sponsorships in their own capacity. These purchases must be made independently and may not be connected to, credited toward, or conditioned on any grant made by the foundation. Ancillary benefits that are available for separate purchase can be listed under the Items for Purchase section of the donation request form.

Donor Recognition

As a 501c3 nonprofit foundation, we cannot accept ancillary benefits. Any benefits received must be fully tax-deductible. Acceptable benefits include things such as printed and digital logo recognition, verbal recognition, and written recognition.
